1️⃣ Insurance & Payment Information
One of the most common questions we receive is:
❓ “What insurance do you accept?”

✔ If you’re unsure about your coverage, please call us at 715-600-2798, and we’ll be happy to help.
2️⃣ Scheduling Your First Appointment
📌 For Mental Health Services: If you would like to receive traditional mental health care and become a new patient, please call us at 715-600-2798 or stop by the clinic to get started.
📌 For Nutritional Consulting Only: If you’re only interested in nutritional consulting, we offer a fast-track option! Please call us to schedule your first appointment.
3️⃣ Getting Established as a New Patient
When setting up your first appointment, please have the following ready:
✅ Basic Information – Your address, phone number, and emergency contact.
✅ Insurance Details – The name of your insurance company and policy number.
✅ Medical Records (if applicable) – If you’re currently seeing a provider elsewhere, request that your records be sent securely to [email protected].
4️⃣ Patient Portal & New Patient Forms
Once you’re established, you’ll receive access to our secure patient portal where you can:
✔ View and schedule appointments
✔ Communicate directly with your provider
✔ Request prescription refills
✔ Complete all required new patient forms (these must be filled out before scheduling your first appointment)
📝 Important: While filling out your forms, be sure to save your progress so your information is stored in our system.
5️⃣ What to Bring to Your First Appointment
✔ Arrive 10 minutes early
✔ A physical copy of your ID and insurance card.
✔ A list of any medications you are currently taking.
